Licensing observed thru BIDS that background check had not been done on person who has moved into the home on Saturday. Visit: Periodic / Full. Plan to correct: Provider stated that she will have her assistant complete background check today. Licensing spoke to Assistant and she stated that she would complete this today. Licensing observed that Nola did not have vaccination records. Visit: Periodic / Full. Plan to correct: Provider states she has dog scheduled for tomorrow to get shots. Primary caregivers licensed or employed on or after June 1, 2022.

Licensing observed that Primary Caregivers PDL level expired on 9/28/2025, Was requested 7/30/25, but provider states she received notification that qualifications were not met. Visit: Periodic / Full. Plan to correct: Provider states that she will complete training and get request new PDL sent in to CECDP. She states that she will complete the training online. CCS designee reported primary caregiver observed to be vaping twice on 10/14/2025 with children in care. Visit: Periodic / Full. Plan to correct: Provider stated that she will not be vaping while children are in care. Provider stated that in the future she will not have vape out or used while children are in care. Plan to correct: Provider states that she will get with vet and schedule vaccinations for all pets. In the future provider will maintain current vaccination records on file for each pet. Licensing observed that two children are missing immunization or exemption forms. Visit: Periodic / Full. Plan to correct: Provider states that she had gotten the exemption but is unable to located form. Provider states that she will request this again and keep form in childrens file. Provider stated that she will also contact other parent to obtain records. In the future provider will maintain childrens records to include current exemptions or immunization forms. Plan to correct: Provider stated that vet has contacted her about this and she will set up an appointment for tomorrow to have vaccinations completed. Licensing observed that neither child present today had immunization record or current exemption form for immunizations. Visit: Periodic / Full. Plan to correct: Provider stated that she will get this done ASAP. Email was sent again to provider from licensing with instructions and form. Hot water temperature is 158' in children's bathroom Visit: Periodic / Full. Plan to correct: Caregiver stated she will adjust hot water heater so hot water temperature is no hotter than 120'. 3 pets were in dining room while children were eating lunch Visit: Periodic / Full. Plan to correct: Caregiver stated she will put dogs outside and cats in a bedroom while preparing and serving food.
