   An individual with an incomplete background request was present working with children when licensing arrived at the program. The background request was submitted on 4/1/25 and the incomplete notice was sent to the program on 4/3/2025. Visit: Periodic / Full. Plan to correct: Staff left facility immediately and will not return until a clear BIDS is obtained. Licensing sent owner a QE and she will fill out and mail in so she has direct access to BIDS going forward. Plan to correct: I will make sure when children are in my care that the schedule is followed and if a child wakes up, I have a room and designated area for the children to go and read a book, play a game, or watch tv based on their age by DHS licensing procedures.
