Total number of children

There were 13 children present on site, the home is licensed for a maximum of 12. Visit: Periodic / Full. Plan to correct: Primary Caregiver will have her child go with a family member if she is going to be at full capacity that day. Two caregivers

There were 13 children 1year old and older present at the program with 2 program staff. Visit: Periodic / Full. Plan to correct: Program will ensure they are maintaining licensing ratios in regards to ages and number of children at all times. Maintenance

A live roach was observed in the office area. a few dead roaches were observed in the bathroom. Visit: Periodic / Full. Plan to correct: Primary Caregiver has had exterminators out once per week to treat for pests. This has been ongoing for 3 weeks. Primary Caregiver will continue to have home treated to rid of pests. Assistant caregiver

At the time of licensing's visit, the Assistant caregiver who has been employed longer than 3 months has not completed first aid and CPR training at the time of visit. Visit: Periodic / Full. Assistant caregiver

At the time of licensing's visit, Assistant caregiver have not obtained and maintained a current Level 1 or higher OPDL certificate. Visit: Periodic / Full. CPR and first aid certification

Primary caregiver has expired first aid/CPR as of 3/31/2025. Assistant caregiver who has been employed longer than 3 months has not completed first aid and CPR training. Visit: Periodic / Full. Plan to correct: Primary caregiver and assistant caregiver will complete first aid and CPR. The substitute present during today's monitoring visit has current first aid and CPR will act as assistant caregiver and remain in the program during child care hours until the primary caregiver has completed first aid and CPR renewal training. Hazards

Bathroom sink cabinet does not have a child-safety lock making cleaning products accessible to children. Visit: Periodic / Full. Assistant caregiver

Assistant and substitute caregivers

Documentation for first aid/CPR was not onsite for Licensing to view for the assistant caregiver. Visit: Periodic / Full.
