Primary Caregiver was unaware there was a back page and states she was not completing the forms correctly nor providing the back page to the parents. Visit: Periodic / Full. Plan to correct: Licensing provided Ms. Hale with the compliance file forms and insurance notifications. Licensing explained to Ms. Hale the process for completing the forms correctly. Ms. Hale printed the forms and will have the parents complete the forms by Friday. Ms. Hale will assure the forms are completed correctly and completed annually. Caregiver failed to notify licensing of a matter that is required to be reported within 24 hours. Visit: Other / Partial. Plan to correct: Child Care Licensing reviewed the policy from the requirements book on site with primary caregiver.
