The center shall retrain all staff on policy. Proof of training is required and all staff signatures will be verified. Regulation: The center shall take any steps required by the Office of Licensing to correct conditions in the building or center that may endanger the health, safety, and well-being of the children served.

All staff must ensure that the children are adequately supervised while in the bathroom, the center shall ensure that the bathrooms are maintained in a sanitary manner and that toilet paper rolls are inside of the toilet paper dispensers. There should not be water on the floor to ensure that no one slips. Ensure that children are directly supervised at all times including At the time of the inspection, a named staff member was observed on her cell phone FaceTiming an individual while in the classroom. The staff member was observed on her personal cell phone. This same individual was one of the staff members who was unable to provide an accurate number of children that were present in the classroom. The center shall retrain staff on the center's policy cell phone usage and center's supervision policy within the next days. The center shall provide proof of training, materials used and all staff signatures. Regulation: The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children. 1.
