The location of the first aid kit and any additional first aid supplies; ii. The name, address and telephone number of the physician retained by the center or of the health facility to be used in emergencies; iii. An alternate indoor location that operates during the same operating hours as the center. The center shall: (1) Select an alternate indoor location within safe walking distance; or (2) Develop a written procedure for obtaining emergency transportation to the alternate location; iv. At the time of the inspection, a 2.5yr old child was observed sitting in a high chair. The child was not eating and the staff member reported that the child is "too hyper" and that she had him sitting in the chair so that he would not "choke." A conversation was had with both classroom staff. It was explained that no child can be seated in a high chair as a passive restraint. The child was removed from the chair during the inspection. The center shall retrain all staff on positive discipline within the next 30 days. The center shall provide proof of training, materials used and all staff signatures. Regulation: (a) The methods of guidance and discipline used shall: 1. Be positive; 2. Be consistent with the age and developmental needs of the children; and 3. Lead to the child's ability to develop and maintain self-control. (b) Staff members shall not discipline children for failing to eat or sleep or for soiling themselves.
