In the infant room, there were no staff on the floor supervising the infants that were on the mat. Additionally, the 2 staff in the room were not aware of a named child's injury and did not see how it occurred. Regulation: The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children. 1. At least one staff member shall be physically present in the room or area in which children are napping and shall be able to summon other staff members without leaving the room or area. 3. A staff who does not work in the infant room had to intervene when a child walked onto the mat with a hard toy and made a swiping motion towards the named child. The staff assigned to the room again did not intervene when that same child returned to the mat after being removed.
