Based on information, ensure that adequate supervision is maintained at all times throughout the center. A named child exited the playground unsupervised and was found alone wandering the street after about 10 minutes. Note, supervision throughout the center was appropriate, however, unable to abate violation of playground supervision, as children were not permitted outdoors due to snow on the playground/10 degree weather. 2/5/25-Note, supervision was observed to be appropriate throughout the center and during indoor gym transition, however, due to snow on the playground, center was not using the outdoor playground. Unable to abate the violation. Regulation: The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children. 1. Based on information, ensure that adequate procedures for tracking children are adhered to at all times throughout the center. Three named staff were not aware that a named child had left the playground unsupervised until the child was returned to the center by the police. Regulation: Handwashing requirements are as follows: the center shall ensure that children three months of age and older wash their hands with soap and running water immediately after coming into contact with blood, fecal matter, urine, vomit, nasal secretions, or other body fluids or secretions.
