Evidence of violation: It was reported that two vehicles were used to transport the children from the childcare to Wahpeton--zoo. Both vehicles that were used aren't in the childcare licensing system to be used as transportation and no insurance/liability verification are given in the childcare licensing system. No written permission forms that parents would need to approve their child could go on the fieldtrip could be shown as per Melissa's transportation policy. Method of correction: Update the childcare licensing system with the vehicles that will be used for transporting the childcare children. Provide a copy of the permission form that will be used for future fieldstrips. A blank line can be used for where the future fieldtrip with be going-- the form should include what insurance coverage is being used and who will be doing the transportation/driving along with what child restraint systems will be used for their child.
