Evidence of violation: The video that was sent by the facility shows exactly what happened. They did notify this licenser within the required time limit. However, the child did receive a minor injury. Method of correction: Attach a safety gate on the multiple step end of the loft to stop children from going down that way. Put up a railing or move a bookcase next to the one step entrance for the children to use to stabilize their step-up motion.
