Correction order — Rule(s) 75-03-10-12(e) - Shall be currently certified within ninety days of employment and prior to staff member having unsupervised access to children under care, in pediatric first aid by a program approved by the department;

Evidence of violation: While reviewing staff qualifications it was noted a staff member did not have Pediatric 1st aid certification with the first 90 days of employment. Method of correction: The staff member can obtain Pediatric 1st aid at www.icpri.com. Staff members must complete Pediatric 1st aid training within 90 days of hire. Evidence of violation: During a review of the outdoor play area, it was noted the fence had an area where there was a large enough gap that children could get through. Method of correction: The fence gap in the fence shall be repaired in such a way that children cannot get through the opening. Fiscal sanction if not corrected: $25 per day. Correction order — Rule(s) 75-03-10-18(10) - 10. The operator shall ensure that potential hazards, such as noncovered electrical outlets, guns, household cleaning chemicals, uninsulated wires, medicines, and poisonous plants are not accessible to children

Evidence of violation: During the unannounced review it was noted an outlet needed to be fixed. There was a gap at the top of the face place and object could be stuck into the open area. Method of correction: The outlet will need to be fix so there is no space between the face plate and the wall where object could come in contact with wiring. Correction order — Rule(s) 75-03-10-26(7B) - b. Medication must be stored in an area inaccessible to children, and medication stored in a refrigerator must be stored collectively in a spillproof container

Evidence of violation: During the visit it was observed in one of the rooms a nebulizer was placed in a sink with a medication bottle next to it. In another room there were medication bottles placed in plastic bag on the counter. Both medications were not properly stored and out of reach of children. Method of correction: All medication must be inaccessible to children and stored appropriately at all times. Fiscal sanction if not corrected: $5 per day. Time allowed for correction: 20 Day. Evidence of violation: During the unannounced visit it was observed there were 9 infants in the infant room. 8 of the infants were sleeping while 1 infant was awake. There was 1 staff present. The room was over ratio. Method of correction: Ratio shall be maintained at all times in an infant room. Fiscal sanction if not corrected: $25 per day. Time allowed for correction: Immediate. Correction order — Rule(s) 75-03-10-27(5) - The owner shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once ever

Evidence of violation: As staff qualifications were reviewed, it was noted that three staff members: Arrianna J., Chyanne M., and Lerynngies T. did not have a current SFN 375 transfer forms completed. A request was made for the current SFN 375 transfer form to be sent via email or uploaded to the system. Later this afternoon an email was received by the Criminal Background Check Unit stating the SFN 375 transfer forms for the 3 staff members had been rejected. Method of correction: Arrianna, Chyanne and Lerynngies will need to complete the fingerprinting process and obtain a cleared background check. The three staff members will not be able to work at the center until the clearances have been received. If there are staff that have come from another site SFN 375 Transfer Forms need to be completed prior to the first day of employment. The forms must be signed and dated by the Criminal Background Check Unit prior to working at the center. Fiscal sanction if not corrected: $25 per day. Time allowed for correction: 20 Day.
