   Evidence of violation: On 12/23/2025 an unannounced visit was made to follow up with CPS report. After interviewing staff members, it was determined that the incident occured when no staff member was present. Method of correction: Childcare facility to ensure proper supervision at all times. Efficient communication with staff members when they are stepping out of the room to do another task and make sure the room has a staff member in the room before leaving.    Evidence of violation: It was found during the Unannounced Visit conduced on 3/4/25, that a staff member was working while having an expired BG results from another facility. Method of correction: Informed the facility that staff member cannot be present in the facility until a new BG/Fingerprinting has been completed and have results from that. Facility to email licensing specialist a copy of new results or upload it onto the portal for staff. Fiscal sanction if not corrected: $25.00 per day. Time allowed for correction: 20 Day.
