Evidence of violation: During annual review process it was noted that staff member E.H. lapsed in Safe Sleep training. A.W, B.N, AND E.M lapsed in Mandated Reporter training Method of correction: Log into the Hub and take Safe Sleep and Mandated Reporter training within allotted timeframe - Create a list for all staff with training expiration dates and ensure staff take training in a timely manner to avoid future lapses. Evidence of violation: During an unannounced visit file review that staff member A.A had a background check that was not determined eligible. Evidence of violation: A concern came in that during pick-up a child was not located for approximately 5-6 minutes, staff did not know where child was when asked. Child was found in another room alone. Method of correction: Ensure safe care for all enrolled children at all times, doing headcounts multiple times during the day and during each transition.
