Evidence of violation: During an unannounced visit conducted on 5/5/26 it was noted during a conversation with Jamad that she purchased a new car seat to fit child Y.B. better, Jamad also mentioned that when his mom (her daughter) is not present she straps Y.B in car seat and bungee cords the car seat to couch. Method of correction: The provider must immediately discontinue use of car seat or any restraining device not designed for behavioral or therapeutic use in childcare setting. Contact USpire for further guidance - contact information was given to Jamad in an email. Fiscal sanction if not corrected: $25 per day. Time allowed for correction: Immediate.
