Evidence of violation: An incident was reported that resulted in the injury of a child due to a lack of supervision during outdoor play time. Method of correction: Complete observations to ensure staff members are adequately spaced throughout the outdoor play area to be in position to intervene in situations to prevent unnecessary injury. Evidence of violation: By SFN 383 report and video of incident, staff member Berlynn Passler was inattentive while a child climbed on and stood on a small chair of a plastic chair and art easel set. Part of the easel is in view in the video but not the child. He is seen walking across the room to staff who were busy with a white board they had placed on the floor. He appears to be crying but the staff does not look up. It can be seen that the child is bleeding and continues to cry. Staff finally looks up. Child fell at 1:52, at 2:01 child is standing near staff, bleeding, crying, wiping face. 2:47 child is acknowledged by teacher who then takes action.
