Correction order — Rule(s) 75-03-10-27(5) - The owner shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once ever
Evidence of violation: During the unannounced visit while going through the staff files it was noted one of the staff members has a fingerprint memo that had expired in February of 2025. This staff member was working and was in ratio. She was asked to leave and not return to work until she has her fingerprint clearance completed. Method of correction: The center will need to give the licenser a detailed written plan on how they will make sure to monitor staff records to ensure they are current and compliant with state and federal regulation.
