Evidence of violation: While at the unannounced visit I asked the staff in charge who the director and supervisor were, the staff stated Audrey Marsh(director) and Leah Locklear(supervisor). Staff are not listed in the system as Director or Supervisor and there has not been a Director/Supervisor change application completed prior to staff becoming the director or supervisor. Prior to the visit I was unaware of any staff changes. Method of correction: The facility must notify the department of any staff changes including new hires and director/supervisor changes prior to the staff working as the director or supervisor.
