Evidence of violation: During my unannounced concern visit, I observed some safety issues in the back fenced-in play area where children were present: 1. Wooden Pallets with Exposed Screws: Multiple wooden pallets were left in the play area, some with screws sticking out posing a serious injury risk. 2. Broken Items: There were various broken items scattered throughout the space, including chair backs, a damaged tote, and several broken toys. (The teacher and children did remove these items when I was there.) 3. Hole in the Ground from children digging: A decent size hole was present in the middle of play area, creating a tripping hazard. Dispose of Broken Items: -Throw away or safely remove any broken toys, equipment, or materials from children's reach. -Do not leave damaged items in or near play areas. Fill in Any Holes: -Fill in any holes in the yard or play area with dirt to prevent trips or falls. -If children express interest in digging, create a designated digging area off to the side, away from high-traffic or hazardous zones.
