A safe indoor and outdoor environment was not provided for the children. In Space G6 a closet which contained staff members purses was not locked and a fan was located on top of a shelf not 5 ft off the floor. In Space G4 a glass window is cracked. The kitchen door located in the modular was unlocked. On the 1 and 2 year old playground the underpinning was broken and damaged exposing shard edges.
