5 CSR 25-400.095 — Sleeping equipment shall be arranged to provide at least a two-foot (2') aisle on one (1) long side of the equipment.

5 CSR 25-400.085 — Concrete, asphalt, carpet or bare soil is not an acceptable surface under outdoor equipment from which children might fall and be injured.

The fall-zone area under and around outdoor equipment where children might fall and be injured was not covered with impact-absorbing materials to cushion a fall as evidenced by bare ground at the base if 2 of the swings. Stationary equipment, green dome climber, was unsafe as evidenced by the equipment was placed in close proximity to other equipment which could cause a collision hazard - the equipment location and measured distance from other equipment was next to wooden swing frame.
