Safe sleep training was not completed within the past 3 years for the provider, Tyria Baker (rec'd 4/10/2026), Kim George (rec'd 4/10/2026), and Eli Holt, Jr. (rec'd 4/10/2026). As of 4/10/2026, safe sleep training was not completed within the past 3 years for Kourtney Burchfield.
