The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by there is a shed in the outdoor space that is slightly raised off of the ground. The shed if you are facing it on the left hand side has a gap that is measuring between 4, 6, or 7 inches along the left side bottom of the shed. This presents a outdoor hazard. Children could potentially crawl under the shed, access the underneath area of the shed or may become entrapped under the shed. Correction required: The outdoor play area shall be clean, safe and well-maintained as required. Inspection type: COMPLIANCE VERIFICATION.
