The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence had openings large enough for a child to crawl or fall through - the opening measured 11" in the southeast corner of the fence, this appears to be a hole dug by an animal. Also a few sections of fencing away from this hole is a hole created when a fence post was removed. Correction required: The outdoor play area shall be accessible without hazard and provide a safe fence around the outdoor play area as required. Inspection type: COMPLIANCE VERIFICATION.
