The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by in the right hand corner of the playground next to the swing and slide and the gate entry orange mesh was observed on the ground with several polls entangled in the mess. The mesh and poles were accessible to children. The mesh is a hazard for tripping and entaglement, the poles were not laying down but were askewed and poking up from the ground.
