Children were running on upper level of the gym not supervised as well as exiting the gym without the educators knowledge. Educators were observed playing basketball with a few children while the remainder of the children were unsupervised. Educators may not engage in activities that unnecessarily that take their attention away from the children. Corrective action: The program has updated supervision procedures to ensure children are closely monitored during gym and large group activities. Educators will remain attentive to all children while participating in activities and will periodically check on children who are not directly involved to maintain a safe environment..
