7.11(13)(e), 7.10(05)(a) — Care of Children

During the visit an infant was observed sleeping in a sleep sack in which the child's arms are not free, and may restrict the child's movement. Corrective action: “Safe Sleep Incident Report – March 26, 2026 On the date of the visit, the child’s sleep sack was removed due to non-compliance with safe sleep guidelines, for the lack of Movement. The parent was notified immediately. The parent subsequently provided an arms-free sleep sack and signed the EEC Safe Sleep form. All staff were informed of the incident and reminded of proper safe sleep procedures. Staff members responsible for the care of the infant will complete Safe Sleep and SIDS training through Strong Start within 10 days. The incident will be reviewed during the staff meeting on March 26, 2026, to reinforce adherence to all safe sleep policies and procedures.. Regulation: Programs serving infants must:, Children younger than six months of age at the time of enrollment must be under direct visual supervision at all times, including while napping, during the first six weeks they are in care. 7.07(07)(f) — Physical Facility - Outdoors

Pea Gravel erodes from the PS playground onto the TODD playground. The program must have a plan in place to remove and prevent further erosion of the pea gravel. Corrective action: We will put up plastic mesh fencing 5mm hole to prevent the pea gravel from spreading. We will be reaching out to a landscaper to complete removal of pea gravel on toddler play area. We will meet Wednesday 11/12/2025 to finalize plan of action.. Regulation: The outdoor play space must not be covered with a dangerously harsh, abrasive, or toxic material. Cited during: Provisional License Renewal Visit. 7.07(13)(d) — Physical Space (Indoor and Outdoor)

Space between the fencing and the lower horizontal metal pole has a several inches of slack that could lead to an entrapment. Corrective action: We will be putting zip ties to bring fence to pole to fix the areas of slack and concern. Completed at 12:30pm today.. Regulation: All play equipment, fences and structures must be free of entrapment hazards. Cited during: Provisional License Renewal Visit.
