Need a detailed health care plan for one child. Licensor emailed a copy of a health care plan on 9/8/25. Do Not Upload Health Care Plan to this visit report. Corrective action: The child who needs the health care plan is not here no more. The last day was 09-18-2025. 7.04(07)(a), 7.04(07)(b) — Administration

Check all enrollment forms for completion. Some are missing the age at admission, physical descriptions, walks in the neighborhood, anticipated days of care, assistant's name need to be added to written parent permission forms and correct date of admission. One child need a lead screening test. Corrective action: I check the enrollment forms and now everything is complete and i add the assistant name. I talk to the parents for the kids who need the lead test, and she told me that is the only one she has. 7.09(04) — Administration

Educator need to add any training completed after 2023. The PQ Registry renewal date is 8/1/2026. Corrective action: I upload my PQR antes my assistance as well,and my 2.0. Daily attendance is missing for February 2025 and there is some missing times on the parent sign in/out sheet. Corrective action: I was looking for the February sheet and i didn't find it.I talk to all my parents about the sing in and sing out and they do it daily.. Regulation: daily attendance records indicating each child’s attendance, including arrival and departure times;, a method of knowing exactly who is present on the premises at any given point in the day. 7.07(06) — Physical Facility - Indoors

The pole on the front porch has peeling paint. Corrective action: the paint is already scraped, now i have to paint them.. Regulation: Chipping and Peeling Paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster. Cited during: Progress Visit. 7.07(07), 7.07(10) — Physical Space (Indoor & Outdoor)

The hot water temp is reading 132.3 degrees. No other indoor hazards observed. Smoke/carbon detectors not tested during this visit some children are sleeping, per educator they are working. Corrective action: I fix the hot water. Regulation: Outdoor Space. The licensee must maintain, or have access to, an outdoor play area of at least 75 square feet per child who is outside at any one time., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning. Cited during: Progress Visit. 7.11(13)(d)(03)(b) — Routine & Activities

The playpens need fitted sheets on them for napping. The children can not share the same bedding. Corrective action: I bought another playpen now i have 3 and they have sheets'.
