7.05(05), 7.05(06), 7.05(08) — Care of Children

During the visit two children were observed sitting in the Preschool Directors office. It was reported they were in the office taking a break due to challenging behaviors. Corrective action: We do acknowledge that there were two children observed in the directors office during the visit, as we know this acting does not align with EEC expectations. Something we will put in place for future scenarios of misbehaving children that need a break are staff training to refresh the staff on positive ways to redirect and guide the child's behavior, and how important it is to keep children in their assigned space. This will also provide alternative strategies for handling and managing challenging behavior. If extra support is needed, after school staff or administrators will also be called to see if they can be of any help or assistance to the situation at hand.. 7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

While observing the classrooms there were microwaves within children's reach. The Licensor suggest microwaves are kept out of reach from the children to prevent injury to children. They can be removed completely or hung up and secured at least four feet from the ground, out of reach from children. The classrooms had teachers desk that were located in the licensed activity space for children. The Licensor suggest they block the desk area off from children, while ensuring it does not run into the required 35 square feet per child activity space, or the remove the teacher desks from the classrooms. Preschool classroom A had dividers between the gross motor room and classroom A that were falling over and not secured to the floors or walls. The Licensor and the Director talked about using shelving and baby safety gates to secure the wall between the gross motor room and Preschool A room. The Director showed the Licensor available shelves to divide the two rooms during the visit. They reported the current unsecured dividers would be removed and the shelves would be secured to make a wall. The preschool chairs had potential head entrapments. The cloth covering on the chairs to eliminate the head entrapments were not secured on the chairs, some chairs did not have the cloth coverings at all. The Licensor suggest looking into other ways to eliminate the head entrapments beside the cloth covering. Or removing the chairs completely and replacing them. Corrective action: A. All microwaves that were seem in children's reach will be removed immediately from the classrooms until the proper microwaves mounts are delivered. Order placed on July 11th. They will be mounted at least 4 feet above the ground and out of reach in accordance with EEC policy. B. Teacher desk in the affected classroom will be completely removed to ensure they do not reduce the required 35 sq/ft for child activity space. Room layouts will be updated and ongoing classroom adults will be ensure continued compliance. C. Preschool purchased furniture to be in compliance with EEC inspector. It was on 7/10/2025. We will also properly and securely fasten all seating covers that proposed risk until replacement. Staff will be informed of this safety concern and will complete a daily visual seating inspection.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning. 7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

The program reported they take children to Dana Park, Sara Jane Pool and Rivers Pool. These spaces have not been inspected by the Licensor yet. The programs playground is all cement. While inspecting the playground during the visit the following concerns were discussed with the programs administration. A climbing structure did not have the required fall zones of 9 inches impact absorbing material under and 6 feet around the structure. The sides of the building and along the fencing of the playground on the playground had large sharp rocks, chunks of pavement, uneven pavement, and large holes which could be tripping hazards and possibly cause injury to children. The fencing along the sidewalk that separates the children from the public was not privacy protected, allowing for potential hazards to children being disposed of on the playground by the public, public people engaging with the programs children while they are on the playground and possibility of breaching confidentiality of children enrolled. Corrective action: The Climbing sturucture is no longer being used and will be put away until the proper impact absorbing material and spacing is put in to follow EEC standards. A contractor and landscaper were scheduled to look ate the play ground area on 7/12/2025. Repair work will schedule between 7/21-7/25 regarding weather conditions. Privacy screening be ordered 7/11/2025 and will be installed as soon as received.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning., All play equipment, fences and structures must be free of entrapment hazards. 7.13(04)(f) — Transportation

While reviewing driver files, two drivers did not have evidence of current First Aid and CPR. The Site Coordinator reported one driver has current First Aid and CPR but they have not printed the card and added it to the file yet. They reported the other driver does not have current First Aid and CPR, but they are scheduled to take the class. It was reported the driver has been transporting children without a monitor that has current First Aid and CPR. This driver has been transporting children without anyone First Aid and CPR certified in the vehicle. Corrective action: Two drivers took CPR and First Aid Course on 7/14/2025. For the future, drivers will be removed from the transporting students if they don't have monitor or qualified in CPR and Frist Aid..
