Diaper changing was observed in the infant/toddler room, and the children and educator did not wash their hands with running water and soap after each diaper change, and the diaper mat was not washed and disinfected after each use. Corrective action: Staff will be re-trained on proper diaper changing procedures, including required hand washing for both children and educators using running water and soap after each diaper change. Staff were also reminded that diaper changing surfaces must be washed and disinfected after every use. Administration will continue to monitor diapering procedures to ensure compliance is maintained.. Regulation: The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. 7.07(16)(e) — Physical Space (Indoor & Outdoor)

Children were observed playing on a dome climber in the preschool playground on the day of the visit, and the depth of the wood mulch in the fall zones measured 1-2 inches. Corrective action: The preschool playground mulch levels have since been addressed and additional wood mulch was added to the fall zones. Moving forward, staff will ensure climbing equipment is closed and inaccessible whenever fall zone depth does not meet compliance standards.. Regulation: Playground Safety. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. 7.10(05)(d), 7.10(05)(f) — Care of Children

While observing the preschool classroom of 17 children on the playground, The Teacher lined the children up at the fence. One child did not get in line, the Assistant Teacher was trying to get that child to go inside with the group. The Teacher took the lined up children and went inside, leaving the Assistant Teacher alone with one child on the playground. Corrective action: Staff was retrained on EEC regulations requiring appropriate staff-to-child ratios and active supervision at all times. At no point should an Assistant Teacher member be left alone with a child outside. Teachers must ensure the group remains together and transitions occur safely and as a team. This incident will be reviewed with the staff involved, and ongoing monitoring will be conducted to ensure compliance. Further violations may result in disciplinary action.. Regulation: Educators must be positioned to maximize their ability to see and/or hear children in their care., Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children. 7.05(05), 7.05(06), 7.05(08) — Care of Children

While observing the preschool classroom outside the children were playing with woodchips. A child told the Licensor they are not allowed to play with woodchips, in response the Teacher told the children to line up and go inside. While walking around the playground the Licensor observed a child punch another child in the back, when the Licensor pointed it out to the Teacher, they did not address the hurt child or the child who punched them. The Teacher in the classroom used the phrase "No thank you" often when they wanted a child to stop something then gave the children no guidance. Corrective action: The teacher was retrained on EEC regulations regarding active supervision, appropriate behavioral guidance, and immediate response to peer conflicts. Staff will provide clear, consistent redirection and address all incidents promptly and appropriately. Classroom practices will be monitored by administration to ensure compliance. Continued failure to meet expectations will result in further disciplinary action..
