Although an educator's first aid training was current, it was not current the entire time they have been employed. Corrective action: First Aid CPR training was scheduled in February 2026 but was cancelled due to weather and rescheduled in March which had those staff with February 2026 expiration dates out of compliance. The program has two scheduled class a year with one in February and one in June. This keeps all staff on a February or June recertification schedule.. Regulation: First Aid Training. Review of 3 staff files found 1 with no evidence of a current physical. It could not be determined if observations are conducted bimonthly on all educators due to no observations being in staff files. Corrective action: The physical in question was sent via email and was not printed out and placed in the staff file. This was found when we did our quarterly audit in June, and was discovered in our quarterly file reviews in April that is needed to be updated as it expired in May 2026. The quarterly reviews are a work in progress and will now have a component that has w do date listed for annual paperwork . AS for the Observations tthey were in with our annual reviews/ job performance evaluations and were being used for performance rating . They have been filed back in the staff individual files.. When Licensor arrived, the program had only two educators with 29 children. Corrective action: The Afterschool Programs had had two staff resign the last week of April and Then two other staff resigned the second week in May creating an unexpected staffing shortage. As of 5/15 we had lined up two additional staff but need to do on boarding and were not ready to start until 6/8/26. Additionally that day we had two call outs and a PTO day for another staff. The plan was to staff WCA with two staff and have a third back up if we went over census . We went over census but had our tutor on site with three children so the back up staff was called in so when tutoring was over we would have three staff. Unfortunately this was the only solution that day. We are currently fully staff as we are prepping for summer camp and qualified staff have been helping cover vacancies.. Regulation: daily attendance records indicating each childs attendance, including arrival and departure times;, The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. A child's [REDACTED]expired 4/2026. Corrective action: The site coordinator failed to double check the expiration on the Epi pen and relied on the prescription date for compliance. The site coordinator immediately checked all the medication with boxed meds to assure these were in compliance. The medication in question was replaced with a non expired epi pen. Going forward both the outside prescription and medication inside the box will be check for expiration dates when received from the parents/guardians.. Regulation: All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturers packaging., Unless otherwise specified in a childs individual health care plan, the educator must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care and during the transportation of children. Educators were not providing adequate supervision of children inside or on the playground. When Licensor arrived and walked into the hallway where the children were waiting to transition outside none of the staff could be seen visibly in the hallway supervising the children. On the playground educators were not positioned adequately to supervise all children in their care. Educators often had their backs to most of the children as they engaged with only 1 or 2 children or were positioned closely to each other when children were at all ends of the playground. Corrective action: Worcester Cultural Academy Site coordinator was notified of these non compliances. The staff were each given verbal warnings and the Site Coordinator was additionally written up and provided a work plan to correct these supervision issues and provide on going support to the WCA staff to assure proper supervision and global Awareness in the program at all times. Regulation: Educators must be positioned to maximize their ability to see and/or hear children in their care., Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children. Corrective action: Rainbow CDC has instituted a plan to make sure Parents /Guardians sign the required documents in a timely manner. Rainbow CDC will be giving parents a two week notice to terminate services two weeks prior to their documents expiring, if they have not responded to previous requests to complete their document before they expire. Corrective action: Rainbow has contracted with a new Payroll/HR/Onboarding vendor and will begin to use a new onboarding process with this company. The process allows for new employees to access the required tasks and documents through their secure portal and staff will not be allowed to start working without all tasks and proper documents completed.
