7.11(01) — Administration

An educator who was working with the children on the day of the visit did not complete EEC Essentials 2.0. The educator reported that they had completed one module. Corrective action: The staff was [redacted] until EEC Essentials 2.0 was completed. Staff completed training on March 3rd, 2026. The certificate has been placed in the staff's file.. 7.10(05)(d), 7.10(05)(f) — Care of Children

There were two educators with 15 children on the day of the visit. On several occasions, one educator left the cafeteria to walk children to the front doors of the school, as the school does not let parents into the building. Children were also observed leaving the cafeteria without the educators' knowledge and were not supervised in the hallway and while using the bathroom. Corrective action: Educators have been communicated to about proper supervision and ratios. Administrator has reviewed policies and regulations with the staff. The staff will continue to monitor children while using the bathroom and will stay in proper ratios when tending to the door for pick up.. Regulation: Educators must be positioned to maximize their ability to see and/or hear children in their care., Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children. 7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

The program did not have a safe and sanitary method for storing children's personal belongings. Coats and backpacks were on the floor near the bench in the cafeteria. An educator's pocketbook was on the table in the cafeteria and accessible to children Corrective action: Individual laundry baskets are being purchased for the site for the children's belongings to be stored in while in our care.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.07(02), 7.07(03) — Physical Facility - Indoors

The building certificate does not contain the use code. Corrective action: New building inspection has been received and uploaded into LEAD. The site has the updated inspection with the proper use code posted on poster board and in binder.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.11(03) — Health

Educators were not aware that a child had food allergies, and required an [redacted]. Corrective action: Administration immediately communicated to all staff responsible for the child’s care of the allergy. Staff were informed verbally and through written communication via allergy list about the child’s allergy and the importance of following all allergy-related health and safety procedures to prevent exposure.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. 7.11(05)(d)(04) — Health

The program did not have an [redacted] for a child with food allergies. The school nurse confirmed that the child has allergies and an [redacted] is available for the child in the nurse's office. Corrective action: Administrator contacted the parent/guardian to complete the proper medication forms and requested a [redacted] be provided for the program by the child's physician. Moving forward, administrator will review enrollment paperwork carefully and will require proper medication and paperwork be completed and received prior to enrolling..
