7.12(10)(a)(01), 7.12(10)(a)(03) — Care of Children

During the observation in the infant room, it was noted that the Individualized feeding schedules for the infants was not available for review. Corrective action: A feeding schedule is posted on the wall in the infant room, detailing each child’s name, feeding times, and specific dietary needs. Feeding times are individualized based on each child’s drop-off time and are adjusted as needed to accommodate their hunger cues and routines.. Regulation: A current feeding schedule must be maintained, documenting use of either breast milk or formula, if applicable, new foods introduced, food intolerances and preferences, voiding patterns, and observations related to developmental changes in feeding and nutrition., Infants must be held while fed a bottle. 7.04(05)(a), 7.04(05)(b), 7.04(05)(c), 7.09(09) — Administration

One newly hired educator’s file did not have evidence of two referrals, orientation, EEC essentials 2.0 prior to working with children, evidence of two MMR’s, as well as other Staff Records documentation. Corrective action: Newley hired employee submitted documentation; 2 references, PE with evidence of MMR and completed her orientation for personel file. However on 4/18/25 she resigned from her postion.. Regulation: documentation that the employee has the qualifications required under 606 CMR 7.09, including, but not limited to, health records, as required by 606 CMR 7.09(11);, copies of licenses, certifications and registrations held, including, but not limited to, motor vehicle operator’s license (if the staff member transports children) and EEC educator qualification;, documentation of orientation, training and professional development, as required by 606 CMR 7.09(15)(d-f), 7.09(16)(c) and 7.09(17)(a), and 7.11(1), 7.09 (18)(d), and 7.09(19)(b),as applicable;, All educators must attend an orientation to early education and care approved by the Department. 7.04(07) — Administration

During a review of four children’s files, at least one child’s physical was expired. Corrective action: Parent notifications were sent out on 4/8/25. This was sent to all parents regarding annual updates and expired forms. The initial requested return date was 4/15/25. A second request has been requested to all parents again, and the submission date is 4/28/25. Failure to submit on this date will result in children being placed on the stop list until the update has been submitted per the regulation.. Regulation: Children’s Records. 7.04(17)(k) — Administration

Staff Meeting documentation was not available for review, and not provided to licensor by the end of the day. Corrective action: Staff documentation logs were assembled to document the required monthly meetings per month. This information will be sent as requested via email to the licensor .. 7.07(13) — Curriculum and Activities

Shelving units are discolored with lots of stains and could use a good cleaning. One wooden kitchen set in the toddler/preschool room was easily tippable upon inspection. Corrective action: The tippable wooden kitchen set was secured in the toddler/preschool room on 4/11/25. The shelving unit was cleaned as well on 4/11/25. Regulation: Safety Requirements for Equipment, Materials and Furnishings. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children. 7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

During the classroom observation it was noted that the shelving units and floors are very stained and discolored. Shredded rugs pose a tripping hazard several areas of floor tiles are discolored and cracked. Corrective action: On 4/11/25 classroom shelving units throughout the center in all classrooms were secured. If necessary, units were bolted or relocated within the classroom . 04/19/25 The center floors were stripped, buffed and re-waxed to remove the built up wax applied by the vendor. This process has hopefully remedied the stains that caused the discolored look of the floors. This will be monitored closely, to determine if additional changes are necessary to prevent this from happening as the glue and wax appeared to have caused the build up.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning. 7.11(07)(h) — Emergency Preparedness

During the file review, the Fire Drill log was not available for review. The Program Director reported that the log would be emailed to the licensor by the end of the day.. The email was not sent. Corrective action: The Fire Drill Log was inadvertantly removed from the center whiile being electronically added to our digital files. As was requested this information will be forwared by email, as of the date of this report 4/25/25. Regulation: The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. 7.11(12)(d) — Health

During the classroom observations, the licensor observed a bucket of toys on changing table, toys were then placed on shelves for children to utilize before being sanitized. Corrective action: The regulation sited on 4/10/25 was reviewed on 4/11/25 with each classroom. At our regular sceduled staff meeting on 4/17/25 it was discussed and again with explanation on the proper procedures for sanitizing toys and the importance of proper storage of materials. During a medication review at least one child's Medication consent form expired in 12/2024. Corrective action: Parent notifications were sent out on 4/8/25. 7.11(03) — Health

During a review of medication and the IHCP's, the licensor noted that one child's individual health care plan expired 12/2024. Corrective action: Parent notifications were sent out on 4/8/25. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. 7.07(06) — Physical Space (Indoor & Outdoor)

During the classroom observations it was noted that on the toddler room walls, there is peeling paint in several areas. Corrective action: 04/19/25 and 4/26/25 The classroom walls that required paiting due to peeling paint were completed on the dates above. The Shelving units that required painting was painted and those that only required cleaning were cleaned.. Regulation: Chipping and Peeling Paint.
