7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

Back room chair head entrapment. Stained ceiling tiles. Chipping paint on heating pipes. Flooring peeled up in door ways. Flooring holes is tiles. Carpet in hallway to bathrooms was worn down torn, covered in dirt, dust and unknown debris. One large fish tank and one large turtle tank in classroom in the children's activity space. One fish tank with no fish in another classroom in the children's activity space. Wooden floors throughout the program have raised nails, cracks in the flooring, no sealant and are porous allowing germs to live in the floors. Shelving units, lockers, dividers and cabinets throughout the program were not secured down. Corrective action: The wooden flooring in the large room, as well as the carpet in the smaller room and hallway, are currently under review with a contractor, and flooring repairs/replacement are scheduled. The ceiling and heating pipes will be repainted with new paint. The fish tank and turtle tank will be removed, and the space will be redesigned to create a safe and usable area for children. Additionally, all shelving units, lockers, and cabinets will be properly secured to the walls to prevent tipping hazards.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.07(01), 7.11(07)(h) — Emergency Preparedness

It was reported by the Site Coordinator that monthly evacuation drills are not being completed regularly. Corrective action: We will first conduct training with the staff, then start a small-scale fire drill this week, and continue to hold fire drills every month without fail.. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. 14.02 — BRC Information

One van driver has an expired BRC as of 10/4/2025. Another van driver has an expired BRC as of 2/9/2025 the BRC was submitted to renew on 3/3/2025 and is pending. Corrective action: All staff BRCs have been newly updated as requested. Going forward, we will ensure updates are completed within the required timeframe and recorded in the staff files to prevent any omissions.. Regulation: EEC’s BRC regulations are designed to ensure the completion of BRCs on individuals subject to EEC’s oversight, including those in receipt of federal Child Care Development Fund (CCDF) funding pursuant to the Child Care Development Block Grant (CCDBG) Act of 2014 at 42 USC § 9858 and funding through the Families First Prevention Services Act of 2017 at 42 USC § 471(a)(20)(D). Further, BRCs shall be performed in accordance with the statutory framework of M.G.L. c. 15D, §§ 7 and 8, 606 CMR 14.00 and consistent with the timelines established by EEC regulations or policy. EEC is responsible for reviewing all disqualifying information for candidates, with the exception of candidates for adoptive and foster parents to the extent authorized by the Federal Bureau of Investigations (FBI) and state statute. EEC may delegate to another Commonwealth entity its authority to complete BRCs through a Memorandum of Understanding or other applicable agreement to the extent permitted by state and federal law. While effective immediately, EEC will implement the provisions of 606 CMR 14.00 in phases and provide updated timetables for implementation within its written policies. Nothing in 606 CMR 14.00 shall be construed to create a private right of action if an EEC licensed, approved or funded program has acted in accordance with 606 CMR 14.00 and any policies adopted thereunder. The red mat in the large room with wood floors was stained and in disrepair. The Program Administrator reported when it is washed with bleach it does not come clean. The wood floors have no sealant anymore, leaving cracks in the floors that have art materials, food and dirt built up. The cubby units are made of pressured wood, they are breaking. The particle wood is exposed and chipping, the units are not sturdy anymore, the black color is peeling off (all rooms). Three children's couches in the room with the red mat are peeling exposing the insides. The room with children 5-14years has a shelf, on the top shelf is a microwave that is in reach of the children and could potentially fall onto a child if pulled. The small classroom for children 5-7years has floor tiles that are cracked and chipping, the floor boards had built up dirt on them. The small room with desks had a bench with a grey cushion it was stained, the rug by the fish and turtle tank was stained. The boys bathroom had a crack on the toilet seat and had cloth coverings on the toilet lid and seat lid. The girls bathroom also had cloth coverings on the toilet lid and seat lid. Corrective action: We will remove the red mat from the mat room floor and replace it with a new carpet and furniture to create various themed spaces that children will enjoy. We will replace the children's cubbies in each room with new ones. We will remove the worn-out leather sofa in the mat room and provide more toys that children need in that space. We will attach bracket or anchor to the microwave and the rack supporting it in the middle room to prevent it from shaking or falling forward, ensuring safety. We will patch the worn-out flooring in the small room where children aged 5-7 stay. We will purchase a carpet cleaner to remove stains from the carpet in the small room and the gray cushions. We will remove the toilet clothr in the small room’s bathroom and replace the cracked toilet seat cover.. The small classroom for children 5-7 years old had chipping paint on the old heating pipes. Corrective action: We will repaint the old pipeline to cover the peeled areas and prevent the rusty parts from being exposed.. Regulation: Chipping and Peeling Paint.
