7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

While observing the bathrooms, the floors were sticky and the tiles around the toilet were loose leaving areas for germs to grow. The matted flooring throughout the program was in poor condition, some areas were broken along the edge, some areas were duct taped and the edge all around was a tripping hazard. The secured rug right outside of the office and in the book area was soiled and stained, some areas were torn. The rugs looked beyond repair or basic carpet shampooing. Corrective action: The director has scheduled an appointment for 12/24-12/28/25 with a company to come into the site and take out the mats and rugs and replace it with brand new flooring. Also, they will fix the flooring around the toilets at that time. The stickiness on the floor was due to staff using too much floor cleaner, so the director went over with them on how much to properly use to clean the floors, so they won't be left with a sticky residue. Moving forward the director will be checking all sites for any repairs or updates that need to be addressed twice a year.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.11(07)(h) — Emergency Preparedness

The month of October was missing from the evacuation drill list. It was reported they did not complete and evacuation drill in October. Corrective action: The director had a meeting with the site coordinators in each room. And informed that fire drill needs to be conducted monthly and recorded in the fire log. Meeting was 11/12/2024. Regulation: The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. 7.11(10)(c) — Health

There were no disposable towels or air dryers in bathrooms during the visit. Corrective action: An electrician will be coming in by 11/26/2024 to install air dryers in bathrooms.. Regulation: The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. 7.07(01), 7.07(13)(g) — Physical Space (Indoor & Outdoor)

During the visit children were using an outdoor space that was surrounded by parking lots and a driveway with electrical units, meters and pipes. The Site Coordinator reported the children were doing a curriculum activity and only outside to collect leaves, twigs and acorns for an art project. While observing outside, the children brought out balls, an educator was holding a football, and the children kept asking to play with the balls. The licensor over heard the educators tell children they were not outside to play, but to collect fall items for a project. The area the children were in had a basketball hoop. During Prior visits to the program the Licensor has made the program aware of the hazards outside and told them the outdoor space they were using had hazards and should not be used unless all hazards were blocked off by a four foot secured fence. Corrective action: The director advised all room site coordinators of the safety issues outside why we can't use it. And to refrain from using it for any manner. Meeting was 11/12/2024. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter.
