    Posted Fire inspection was dated 11/6/23. The Posted Building inspection certificate had an expiration date of 11/30/25. Corrective action: We attempted to have both the Fire inspection and Building inspection certificates addressed prior to the expiration dates listed on the posted certificates well in advance. We did this by having on going conversations with Most recent conversation via email was with [redacted] on 2/24/26 stating the inspections were completed last week and that [redacted] is currently waiting for [redacted] to issue the certificates of occupancy. [redacted] was unsure if [redacted] had planed to email them but as soon a [redacted] had them [redacted] would be sure to forward them to us. Previous conversations: Email exchanges were forwarded (1/27/26) ahead of Site visits conducted, to my licensor to show the ongoing attempts to reach [redacted]. 1/22/26 email with [redacted] 12/9/25 [redacted] 12/4/25 [redacted]. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Fire Inspection. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
