7.07(02), 7.07(03) — Physical Facility - Indoors

The program’s posted Building Inspection had an expiration date of 11/30/25. Corrective action: We have Been working on the Building Inspection for months. I have forwarded every correspondence between myself and [redacted] to my licensor so that [redacted] was able to see the ongoing requests to rectify in a timely manner this inspection. Our most recent conversation with [redacted] was on 3/5 indicating [redacted] would get them to us Friday or Monday. Previous correspondence occurred with [redacted] on 1/22 when [redacted] indicated that all WS expire on 11/30 every year. It took till May of last year to be able to issue a certificate. These ongoing emails were sent to the Fire Chief as well to secure a timely fire inspection. Meeting were held in January with the Principals in a an attempt to work together collaboratively on securing these documents in a more timely manner.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Fire Inspection. Every program not located in a residence must submit evidence of compliance with applicable fire codes. 7.11(03) — Health

The IHCP for a child with a medical conditon did not include the name and telephone number for the Health Care Parcticioner or the parakeet signature and date. Corrective action: We were working with the member's family to secure medical documentation for the diagnosis that was entered into our portal. At the time of enrollment their was limited information available for the diagnosis and not medication required. We had the parent/guardian provide as much information for the diagnosis that [redacted] was able to provide, the only care plan associated was if member complains that [redacted] doesn't feel well to call parent'/guardian to be picked up. We stayed in contact with the parent/guardian to obtain formal documentation from the primary care physician. We have done so and the IHCP has been signed by the parent and the name of the Child's health care practitioner name and number has been added to the IHCP.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
