Review of Staff Records checklist revealed that two Educators [redacted] had expired CPR certification. Corrective action: The program fixed the information on the staff records checklist and updated it with the correct dates to accurately represent the staff's current certification.. On the day of the visit the program did not have an Individual Health Care Plan for child with a chronic medical condition that requires rescue medication. Corrective action: A new IHCP was placed in the child's file on 9/3/2025 to replace the other medical documentation that was present.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The administrator listed on the staff schedule is not on site every day during the hours of operation as required. Corrective action: The administrator's name has been updated on the schedule and the educator is on site each day during the hours of operation. An updated staff schedule was uploaded into the portal to represent the administrator for the site.. Regulation: The licensee must ensure that the program is soundly administered by qualified persons designated with specific administrative and program responsibilities. On the day of the visit the attendance record did not include a child that was present at the time of the visit. Corrective action: Staff were retrained on attendance record keeping and agency policies around supervision. Acknowledgment of this retraining can be found posted in the classroom.. On the day of the visit there were some children awake during nap time. The children were not engaged in quest activities during nap as required by EEC. Corrective action: Educators were retrained in the procedure for providing activities for children who are awake. Children will be given a choice of toy or manipulative to use during rest time if they choose not to rest.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The mat used for diapering children was stored within reach of children on the day of the visit. Children’s bedding was stored touching each other which could create a cross contamination issue. Corrective action: Agency ordered a storage cabinet specifically designed to store mats and prevent cross contamination.. Regulation: The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. 7.11(03) — Health

The individual Healthy Care Plans for two children did not include the names of educators trained and that would administer treatment. The same two children who have multiple chronic medical conditions did not have IHCP for the other conditions. Corrective action: The two individual health care plans which did not have names of educators who are trained to administer the medication has been updated and reflects those staff who were trained to administer the medication. Children were allowed to sit at the tables wile the surface was still wet with a disinfecting bleach and water solution. Corrective action: Staff were retrained in the procedure for cleaning and disinfecting tables or food surfaces.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags and purses are locked or inaccessible to children.
