7.07(02), 7.07(03) — Physical Facility - Indoors

On the day of the visit the Fire and Building Inspections had expiration dates of 2/28/26. Corrective action: Applications for both inspections were submitted in advance of the expiration date; however, due to limited inspector availability within the town, scheduling delays occurred. Despite these circumstances, we recognize the importance of maintaining compliance at all times. To prevent this from occurring in the future, we have implemented the following procedures: Inspection renewal timelines will be tracked with internal reminders set at 90, 60, and 30 days prior to expiration. Applications will continue to be submitted well in advance, with additional lead time built in to account for potential municipal delays. We will maintain ongoing follow-up communication with town departments and document all outreach efforts. If delays persist, we will proactively seek guidance from local officials and EEC to identify acceptable interim solutions or documentation. These steps will ensure that inspections are scheduled and completed in a timely manner and that any delays are actively managed and communicated.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

Inspection of the playground revealed the following hazards: 1) The Geo Dome climber on the playground was not anchored and could tip over. 2) The Little Tykes climber had a crack on the side of the slide. 3) The large climber had chipping paint, rust and exposed anchors on the base of the supports posts. Corrective action: All identified issues have been fully addressed. The Geo Dome climber has been properly secured, anchored, and tested to ensure it is stable and safe for use. The Little Tykes climber has been repaired, with the cracked area securely fixed and reinforced to ensure safe use. The large climber has been repaired, with chipping paint and rust addressed and all exposed anchors corrected to eliminate any safety risks. To ensure ongoing compliance and prevent future concerns, the following procedures have been implemented: Daily visual playground safety checks will be conducted by staff prior to use. Monthly documented inspections will be completed by administration to identify any wear, damage, or safety concerns. Any damaged or unsafe equipment will be removed from use immediately until repaired or replaced. Preventative maintenance will be scheduled regularly to address wear and tear before it becomes a safety issue. Staff will be trained to promptly report any hazards or concerns. These procedures will ensure that the playground remains safe, well-maintained, and in full compliance moving forward.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning., All play equipment, fences and structures must be free of entrapment hazards. 7.07(07), 7.07(10) — Physical Space (Indoor & Outdoor)

Several ceiling tiles were missing leaving exposed insulation in the ceiling of the Infant/Toddler room. Corrective action: This condition occurred as a result of work completed in the classroom while children were not in care. The issue has since been fully corrected, and all ceiling tiles have been properly replaced, ensuring that no insulation or ceiling materials are exposed. To ensure this does not occur in the future, the following procedures have been implemented: Any maintenance or repair work will continue to be scheduled outside of program hours whenever possible. After any work is completed, staff and administration will conduct a thorough safety check of the area prior to children re-entering the space. Classrooms will not be used until all work is confirmed to be fully completed and safe. Staff will be trained to immediately report any environmental or safety concerns. These steps will ensure that all classroom environments remain safe, complete, and in full compliance at all times.. Regulation: Outdoor Space. The licensee must maintain, or have access to, an outdoor play area of at least 75 square feet per child who is outside at any one time., Indoor Space. 7.11(02)(j), 7.11(02)(l) — Health

Review of medication consent revealed that the program did not have a medication consent required by an Individual Health Care Plan. A medication consent form for another children with IHCP was not singed by the parent. Corrective action: [redacted] had the parent sign the form.
