7.07(02), 7.07(03) — Physical Facility - Indoors

The building inspection was expired. The Administrator reported the inspector came and they are just waiting on the certificate. The Fire inspection was expired. The Administrator reported the Fire department said they would be there in January 2026 for the inspection. Corrective action: I reached out to fire department to get inspection done. The person who does inspections is [redacted] so they are going to have deputy do the inspection. I’m waiting for that person to call me and they said it should be in a day or so. Once I get in touch with the person it should be only a matter of a day or so. I do expect to have it done no later than January 23,2026 at the latest. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Fire Inspection. Every program not located in a residence must submit evidence of compliance with applicable fire codes. 7.13(04)(j) — Transportation Safety

The transportation attendance log did not reflect the van driver inspects the vehicle at the end of each run to ensure that no child is left. Corrective action: We always did head count as children left vehicle! We now have new logs that we are using that we will sign off that all children are out of vehicle so we have record of it. Regulation: the driver of the vehicle takes attendance before and after each trip and conducts a complete vehicle inspection after every trip to ensure that children are not left alone in a vehicle at any time; Cited during: Renewal - Monitoring Visit.
