The program administrator reported that mats had been removed from a wall in the older school age room. There was broken plaster and chipped paint on that wall, and small areas on the walls throughout the facility. Corrective action: The damaged areas were addressed immediately. The affected wall in the older school-age classroom was repaired and repainted, and chipped paint observed in other areas of the facility was also repaired and painted to ensure that all surfaces are safe, clean, and in good condition.. The missing signatures have now been obtained and placed in the children's files. The licensee did not properly maintain the outdoor space. There is trash on the ground, standing water on equipment, a hazard covered by a tarp and accessible to children. We blocked it with a shed as we did before and will install safety fence for a gap.. 7.07(06) — Physical Facility - Outdoors

The licensee did not maintain the interior of the program in good repair. There is rust on both sides of the bottom of the door frame and on the floor as you enter the facility. There is one area on the wall in the book center and small spots on the cinderblock walls with chipped or peeling paint in the preschool room. There is chipped or peeling paint and broken plaster on the walls in every room and in the hallway used by the school age children. Corrective action: Door/stairs/cleaning are the building owner's responsibility and we are working with them to keep the stairwell area clean. The building owner will not fix it but provide paint so our facility coordinator has started to prepare for the rust removal and painting. It was observed in all school age rooms that the indoor materials and equipment were not sufficient or visible to the children. There were additional materials stored in a locked closet; however, they were not accessible to children. In the board room, children were watching YouTube, instead of appropriate educational programming.
