7.11(03) — Health

The files for a couple of children diagnosed with chronic medical conditions had IHCP that were outdated on the day of the visit. Corrective action: All paperwork has been updated.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. 7.07(02), 7.07(03) — Physical Facility - Indoors

On the day of the visit the Building Inspection did not include the use codes, and the Fire Inspection had an expiration date of 10/22/24. Corrective action: Licensee has reached out to the building department and asked for updated inspection with codes. Director has reached out for a new fire inspection.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). A Staff Schedule was not available for review or posted to the documents library at the time of the visit. Please upload to documents library. Corrective action: This visit was done for a temporary move for the month of July. We are not in that building until 7/7/25 at which point all of the information will be uploaded. Regulation: the Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors., All educators must possess the qualifications required for their positions in accordance with EEC regulations. 7.07(02), 7.07(03) — Physical Facility - Indoors

The Building Inspection submitted for this temporary location, within a local public school, did not include the Use Code for School Age Care. On the day of the visit the program did not have evidence to show that a Fire Inspection has been completed at the temporary site, located in a Public School. Corrective action: Chicopee Public Schools do not have any use codes listed on their building certificates. We are in the process of obtaining a new certificate by 7/2/25. The fire inspection was done, however no letters were sent to the schools to confirm the inspection and they were not listed on the building certificate either. The fire dept. is scheduled to visit and inspect Bellamy School on 7/1/25.. Regulation: Building Inspection. The program has not submitted an outdoor sketch of the outdoor space. Please upload to the documents Library. Corrective action: This visit was done for a temporary move for the month of July. We are not in that building until 7/7/25 at which point all of the information will be uploaded.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning., All play equipment, fences and structures must be free of entrapment hazards. The program did not have a list of the names of children with allergies or medical information posted in a manner that protects the privacy of children and families. Corrective action: This visit was done for a temporary move for the month of July. We are not in that building until 7/7/25 at which point all of the information will be posted.. 7.07(01), 7.11(07)(h) — Emergency Preparedness

The Emergency Evacuation procedures was not posted on at the time of the visit. Corrective action: This visit was done for a temporary move for the month of July. We are not in that building until 7/7/25 at which point all of the information will be posted.. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. One Epi required by IHCP had expiration date 11/24. Corrective action: The parent was notified and has a new Epi pen.. Regulation: All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer’s packaging., Unless otherwise specified in a child’s individual health care plan, the educator must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care and during the transportation of children. One chide with meds required by IHCP did not have parental consent to administer. Corrective action: We are working diligently to get all signatures on medical forms. That child's parent will be given a new form to fill out and sign..
