7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

Cars are able to access the space behind the building where children play basketball. Corrective action: We typically do not have any traffic in the back parking lot. However, if a vehicle needs to enter or exit while children are playing, staff will immediately collect all balls and equipment, and children will be directed to move to the back of the grassy area. This ensures they remain at a safe distance from any moving cars.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. 7.13(04)(b) — Transportation

During a review of- the driver's files, Driver YJ does not have evidence of a current 7D license. Corrective action: The drivers license will be uploaded to lead. The issue was on the registry end where they didn't send out the new license. We do have the evidence of expiration from his portal. The current license expires on July 17,2025.. 7.13(06)(c) — Transportation

During the visit, the licensor observed a ten year old child seated in the front passenger seat. Corrective action: The routes have been changed so no child will we in the front seat.. 7.11(03) — Health

One child has documented evidence of a milk, egg allergy and has a prescribed epi-pen. The program does not have an IHCP documenting the chronic medical condition. Corrective action: I spoke to mom and she is contacting her doctor to get us updated doctors orders.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
