7.11(01) — Administration

Program staff reported that they are working on the Essentials 2.0 trainings but have not yet completed them. Corrective action: We will ensure all staff has taken the proper training.. 7.04(05)(a), 7.04(05)(b), 7.04(05)(c), 7.09(09) — Administration

The staff records checklist was not current and did not reflect all staff members who are currently working at the program. Corrective action: We have updated the staff checklist and it is current. We will ensure we keep the checklist updated. Regulation: documentation that the employee has the qualifications required under 606 CMR 7.09, including, but not limited to, health records, as required by 606 CMR 7.09(11);, copies of licenses, certifications and registrations held, including, but not limited to, motor vehicle operator’s license (if the staff member transports children) and EEC educator qualification;, documentation of orientation, training and professional development, as required by 606 CMR 7.09(15)(d-f), 7.09(16)(c) and 7.09(17)(a), and 7.11(1), 7.09 (18)(d), and 7.09(19)(b),as applicable;, All educators must attend an orientation to early education and care approved by the Department. 7.04(17)(p)(04) — Emergency Preparedness

Written emergency/evacuation procedures were not posted at each exit. Corrective action: The Hub will reach out to the Site coordinator and make sure all exits have written emergency/evacuation procedures posted.. 7.11(03) — Health

1) File review showed that the program did not have parental consent for a child 9 or older to carry their own medicine. 2) The program did not have a back up supply of the medication for use as needed for children that carry their own medication. 3) A child's medication consent form was missing the parent’s signature. Corrective action: 1)The Site Coordinator has reached out to the parent and has informed them that we need their consent for their child to carry their own medication. 2) The Site coordinator will ask the parent for a back up supply of medication to have in our program. 3) The Site coordinator will make sure the parent signs the medication consent form.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. 7.11(05)(d)(04) — Medication

The program was missing a child's emergency medication according to their IHCP. Corrective action: The Site coordinator will reach out to the parent of the child that is missing medication and will be asked to provide it to our program..
