1) File review showed that the program did not have parental consent for a child 9 or older to carry their own medicine. 2) The program did not have a back up supply of the medication for use as needed for children that carry their own medication. 3) A child's medication consent form was missing the parent’s signature. Corrective action: 1)The Site Coordinator has reached out to the parent and has informed them that we need their consent for their child to carry their own medication. 2) The Site coordinator will ask the parent for a back up supply of medication to have in our program. 3) The Site coordinator will make sure the parent signs the medication consent form.. Regulation: Individual Health Care Plans The licensee must maintain as part of a childs record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the childs health if the treatment is not administered.
