While reviewing an Individualized Healthcare Plan (IHCP) it was missing the parent signature. Another IHCP reviewed was expired. Corrective action: The site coordinator will review paperwork, upon receiving it, for all required signatures. Missing signatures will be requested at the time they are noted to be missing. Paperwork will be reviewed at least monthly by the site coordinator for dates that have expired. Parents will be asked to renew any paperwork found to have passed its expiration date.. One [redacted] was missing the original box and the prescription label. The medication consent form said repeat [redacted] after ten minutes if needed, there was only one [redacted]. Corrective action: The site coordinator will not accept medication without the original container or prescription label. Medication instructions will be reviewed, and additional medication will be requested as needed according to the medication consent forms.. Two children with an IHCP and prescribed medication were missing parental consent for administration of medication. A child with a Peanut allergy was missing required information on the Individualized Healthcare Plan. During the file review educators did not have evidence of a completed BRC. Some educators had the consent form for the BRC. While talking to the educators about children listed on the posted Allergy list. The educators were unsure what child had diabetes and had insulin at mealtimes and had medication onsite. Another child had an allergy and was prescribed an epi-pen and Benadryl. The educators reported they did not know who had the Benadryl prescribed. Corrective action: On Friday, June 6, 2025, [redacted] did a medication review in-service for the Bridge Street School program staff members. At this time she reviewed the EEC paperwork, the medications and the youth who need the prescribed medication. During monthly staff meetings, site coordinators will review paperwork and procedures for youth who have medical concerns at the program. Staff who sub and or are hired midyear, Site Coordinators will incorporate a review of students with medical needs. A bottle of Benadryl was open in a zip lock bag, the bottle did not have a child's name on it. Corrective action: On 6/9/25, [redacted] contacted the parents of the youth with the Novolog insulin and epi-pen signed to obtain parental consent forms. As School Age Child Care registration takes place in the spring, the Director will create a list of youth who need parental consent and health care plans after they review registration paperwork. The Senior Program Director will audit the Director's list before distributing the lists to Site Coordinators..
