7.07(02), 7.07(03) — Physical Facility - Indoors

During a review of the required postings it was noted that the Building Inspection is expired. Corrective action: The Director of Operations has indicated that the building inspection is scheduled for the week of 4/27. We are awaiting completion and updated documentation so the current inspection can be posted and uploaded once received.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.13(04)(j) — Transportation Safety

During a review of the transportation attendance log it was noted that the program does not document that drivers inspect the vehicle at the end of each run to ensure that no child is left behind. Evidence of a second signature inspecting the vehicle is also required. Corrective action: We have informed the transportation monitors and drivers that, where applicable, drivers must sign the transportation attendance log at the completion of each run to document that the vehicle has been inspected and confirmed clear of all children. In addition, the program director will be available in the evenings when drivers return to review the logs, verify completion, and provide the required second signature to confirm that the inspection was completed appropriately.. 7.04(07) — Administration

The program reports that the children's files are not maintained at this site. Files are located at a separate administration building. Corrective action: At present, the children's files are maintained at a separate administration building and checklist is available digitally. We have now printed the relevant Children's Record Checklist and placed it in the visit binder, which is readily available for the Director to access during the visit. We are in the process of reviewing and improving our record-keeping system to ensure that all required documents are easily accessible on-site in the future.. Regulation: Children’s Records. 7.04(18)(d)(01) — Administration

During the visit it was reported that the staff records checklist was not available for review. Corrective action: During the visit, Staff Records checklist was available for review digitally. We have since ensured that the checklist is printed and placed in the visit binder, making it easily accessible for the Director and reviewed the contents of visit binder and digital filing system with Site Director to ensure they can locate and access these during a visit.. 7.11(03) — Health

During a review of four medications, it was noted that the IHCP’s were not available for review. Corrective action: Program has reached out to the families directly to request that the IHCPs be completed and returned to the program by the parents. Additionally, we has conducted an internal review and is developing written process guidelines to ensure that all required documents, including IHCPs, are properly rolled over and updated at the start of each new program season. This will help ensure that files submitted during other program seasons are appropriately completed and maintained for the afterschool season. Moving forward, we will closely monitor the collection and availability of IHCPs to ensure full compliance.. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. 7.11(02)(j), 7.11(02)(l) — Health

During a review of four medications it was noted that the Medication Consent forms signed by parents/guardians were not available for review. Corrective action: The program has reached out to families to ensure that the required Medication Consent forms are completed and returned to the program by the parents. Additionally, an internal review has been conducted to verify that all medication-related documentation is properly maintained and updated at the start of each program season. This will ensure that all necessary forms are available and accessible during future visits. Moving forward, the program will implement a more thorough tracking process to ensure full compliance with medication consent requirements..
