7.10(05)(d), 7.10(05)(f) — Care of Children

During licensor observations a child was screaming, hitting, throwing things and taring up papers. Several educators tried to calm the child down by talking with them. At one point the child left the licensed space and was alone in the vestibule. The educators allowed the child to be alone in the vestibule out of sight from educators because the child was calm in there and liked it. The vestibule is about four feet by five feet and has two doors that open to the outside facing Jackson street. Corrective action: On 12/4/25, staff were reminded that children must remain within sight and hearing of an educator at all times, regardless of behavior or attempts to self-regulate. An appropriate calming space was identified within the licensed, supervised space for future use. The vestibule was designated a non-child area, and staff were instructed that children may not enter that area unless accompanied by a staff member.. 7.11(02)(c), 7.11(02)(e) — Health

One child missing [redacted]. One child missing [redacted] box and prescription label. Corrective action: On 12/10/25 a new [redacted] with the full prescription label attached has been obtained and is now stored according to EEC requirements. On 12/5/25 the Site Coordinator confirmed the [redacted] was on site and located in the counselor’s binder, where it is stored.. Regulation: All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. One child with an [redacted] allergy had an incomplete IHCP. Corrective action: The parent was contacted and the IHCP was completed in full, signed by the parent, and returned to the program. The updated IHCP has been added to the child’s file. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. Corrective action: On 12/4/25, the Site Coordinator confirmed the [redacted] was on site and located in the counselor’s binder, where it is stored. Staff received a refresher on the location of all emergency medications, proper storage procedures, and expectations for maintaining accurate knowledge of which children require [redacted] or other emergency care items.. The building and fire inspection were expired. The program had a letter stating the school failed the inspection, and needed to make corrections then schedule an appointment for a reinspection. Corrective action: The update fire inspection certificate was uploaded into the document library on 10/29/24.
