7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

On the day of the visit the impact absorbing materials appeared to be compressed. Some areas under and around fall zones did not have an adequate depth of impact absorbing material. Corrective action: First thing tomorrow morning, when the heat is bearable, we will rake the loose mulch back under the features and fall zones. Within one month, we will have the mulch tilled to ensure the material is not overly compressed. Moving forward, we will ensure our program team is monitoring and communicating the mulch conditions, we will have our maintenance team is raking the mulch monthly, we will till as needed, and replace mulch when necessary.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning., All play equipment, fences and structures must be free of entrapment hazards. 7.04(04)(a)(01), 7.10(01), 7.10(02), 7.10(03), 7.10(04), 7.10(09) — Care of Children

Red group attendance was lost and one child was not signed in to the group on the day of the visit. Corrective action: Remedial training was provided to all staff that are assigned to the Red group, to ensure attendance sheets are being managed appropriately, and that all members are being tracked when entering and exiting the group. Attendance sheets are being updated to include a section for staff to sign in and out as shifts change. This will ensure another touchpoint for the attendance sheets. When completed, all staff will be trained on the proper usage of said sheets. This will be complete prior to the start of the new school year.. Regulation: daily attendance records indicating each child’s attendance, including arrival and departure times;, The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. 7.07(02), 7.07(03) — Physical Facility - Indoors

Fire and health inspections were not updated on the day of the visit. Corrective action: Both inspections had been completed prior to the visit. Health Department certificate was posted that day. Fire Department certificate was received a few days after the visit, and posted. Both are uploaded here. And both have been uploaded to the Document Library. We will ensure timely receipt of these documents moving forward.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR).
