7.13(04)(j) — Transportation Safety

While reviewing the vehicle attendance two people did not sign off on the post trip inspection to ensure no children were left behind. Corrective action: Moving forward, to ensure we meet 100% of EEC safety requirements, we are adopting the following "Three-Point Check": The Physical Walk: The driver and the second staff member will walk to the back of the van together. The Immediate Log: The log must be signed while inside the vehicle or immediately upon stepping out—not once you reach the building. The Admin Handoff: Upon entering the building, the log must be placed in the "Completed Transport" tray on the Office Manager’s desk. The Office Manager will check for signatures immediately. If a signature is missing, the staff will be called back to the office to complete it and receive a verbal reminder.. While observing the classrooms there was chipped paint in Pre-k 1 under air conditioning unit on the base boards. There was chipping/peeling paint on the walls of the School-Age Art Room. There was chipping/peeling paint on two walls of the School Age Blue room. Corrective action: 1. Immediate Corrective Actions Taken: Pre-K 1: On 4/17/26, the baseboards under the air conditioning unit were scraped, sanded, and repainted with non-toxic, lead-free paint. The area was thoroughly vacuumed to ensure no debris remained. School-Age Art Room & Blue Room: Maintenance has repaired the affected wall sections. Any loose or peeling paint was removed, and surfaces were refinished to be smooth and easily cleanable.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Evacuation procedures must be posted next to each exit in all the daycare areas. 7.11(02)(l) — Medication

Written parental consent for medication administration was missing for multiple children. Corrective action: We have immediately reviewed the medication administration records for all children. We will contact the parents/ guardians of the children for written consent is missing and obtain the necessary documentation. We will implement a revised procedure to ensure that written parental consent is obtained and documented before any medication is administered. All staff will be retrained on this revised procedure at our upcoming staff meeting on 4/18/25. 7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

1) It was not possible to determine whether the equipment meets CPSC standards. The director reported that the material used under the climbers is rubber bonding mulch. 2) The recommended age for the toddler playground could not be verified. The existing playground equipment did not appear to be age-appropriate for toddlers. 3) Documentation was requested from the program regarding the ground material used in the fall zones and the specifications of the playground equipment. The Licensor suggested that the program submit documentation related to the playground and fall zones, or alternatively, a playground inspection report and certificate from a certified playground inspector. Corrective action: We will contact the manufacturer/installer of the playground equipment and rubber bonded mulch verifying that it meets current CPSC standards. We will also obtain documentation to ensure the toddler playground is age appropriate. On 10/10/25 Playsite Services will be coming out to put 2 more feet of bonded mulch to be put around the playscape to be in compliance. We have installed a fence dividing the playscapes to ensure we are in compliance. We have called Cedar works Playsets and they will provide plaques to be put on each playscape showing the age requirements.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space.
