   In classroom preschool A there was clutter in the corner of the room, on top of the fridge and cubbies, the fridge had spills inside it. The toddler room had a few areas of concern that need attention, a mat is torn and needs to be replaced, the cabinet in the classroom has old spills on it and the bottom of the cabinet has water stains on it and appears moldy, there were spaces between the wall and the floor that had dirt and old food in between the area. The licensor sent pictures to the director so the areas could be identified and shared. There was a cabinet that has a lot of debris and dirt caught under it. Corrective action: Clutter was removed from the fridge and top of cubbies. Shelf was organized in the corner. The mat was replaced with a brand new one. The cabinet in the classroom was cleaned with soap and water and bleach. Painted with water resistant paint. Under the cabinet was repainted with mold killing paint after the area was cleaned. The space between the wall and the floor was all cleaned up. Tile was put down that reaches to the wall now so there is no space. The cabinet was moved completely and cleaned the debris and dirt.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning.    7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

   The fence in the infant/toddler yard had one area that is a head entrapment and possible escape issue due to the fence being loose and able to stretch out. The slide in the preschool yard that is attached to the climber measured 12 inches from top of slide to bottom of the protective surface (it needs to measure 11 inches). There were three metal circles attached to the gate between the preschool and toddler play yard that could pose a risk to children. Corrective action: We attached wood to the bottom of the fence to prevent head entrapment and possible escape. We moved woodchips back to the bottom of the slide to measure 12 inches. As the children come down the slide they kick the woodchips out of the way. We will move them back daily. The metal circles attached to the gate were removed.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning., All play equipment, fences and structures must be free of entrapment hazards.
