   7.07(16)(e) — Physical Facility - Outdoors

   There is a slide in the preschool playground that requires proper fall zones. The licensor discussed required fall zones for playground climbing equipment. Corrective action: Slide was taken apart and removed from playground until adequate pea stone depth is delivered. Also, we will be removing sand box completely from the middle of the playground and putting a raised flower bed against the building for children to use as a small sandbox.. Regulation: Playground Safety. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy.    7.11(02)(j), 7.11(02)(l) — Health

   1) The IHCP and medication consent forms for two medications for a child were missing the parent signatures (written parental consent). 2) The program did not have an IHCP on file for a child with food allergies. However, the program did have the necessary medication consent forms and emergency medication for the child. Corrective action: Parent signed all required documents and were placed in the child's file..    7.11(02)(e), 7.11(02)(c) — Health

   A child's epi-pen was expired. Corrective action: This was addressed during the visit, the licensee contacted the parent in the presence of the Licensor to request the new medication. The parent stated that she has the new medication and will be bringing it to the program the next day when dropping off the child. The child was not in care at the time of the Licensor’s visit. Both the parent and the director stated that the parent had come to the center to drop off the new medication and retrieve the expired one. The parent explained that she became distracted while talking and may have inadvertently confused the medications during the exchange.. Regulation: Unless otherwise specified in a child’s individual health care plan, the educator must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care and during the transportation of children., All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.
